Bandwidth.com offers an open access PPO plan. Open access means that you do not need to select a primary care physician, nor do you need a referral to visit a specialist. You have the option to use both in and out-of-network benefits. As long as the providers are in-network then you will be covered at the higher benefit level. If you elect to go out-of-network, then your benefits will be reduced and you may be subject to balance billing by the medical provider.
Bandwidth.com offers a 401(k) Savings Plan after 2 months of employment. Enrollment in the 401(k) program is done at the beginning of each quarter following your initial 2 months of employment. Bandwidth.com will be matching up to 3%, dollar for dollar, with a 5 year vesting schedule.
As part of our group medical insurance plan, you are provided with your own Health Reimbursement Account (HRA). Should you incur an expense where the deductible applies, you will be reimbursed for part of the new higher deductible, and your reimbursement will be tax-free. In addition to lowering the cost of our health plan, the higher deductible plan will also decrease the monthly payroll deductions taken for any dependent coverage.
Your Guardian dental plan does have a network, however the benefits are the same in and out of network. The only advantage to visiting an in-network dentist is that those dentists have agreed not to balance bill members for any amount over the Usual and Customary levels. You will find a list of the participating dentist on the Guardian website below.
New employees will be effective on the 31st day of employment providing that they have completed a Guardian enrollment form during the 30 day probationary period.
Bandwidth.com covers all full-time employees with a long-term disability policy.
Bandwidth.com provides you with $25,000 of life coverage on a non-contributory basis. Non-contributory means that 100% of the premiums are paid by Bandwidth.com. The Basic Life and AD&D policies are payable upon death to any person you name as the beneficiary. You will designate a beneficiary when you originally enroll, and have the option to change this designation at any time. AD&D refers to death by accident, in which case your benefit amount would double.
You and your family have access to an Employee Assistance Plan (EAP) through Unum Provident at NO COST, whatsoever. The EAP is an employer-sponsored assessment and referral service that gives employees and their covered family member's confidential, individual assistance with a wide range of personal and work-related issues.
Employees are able to direct pre-tax dollars to a separate account for non-reimbursed medical expenses or childcare each calendar year.
